Effect of Acute Smoking on Diastolic Function

Abstract: Background: Smoking is a known risk factor of atherosclerosis, endothelial dysfunction, athermanous plaque rupture, unstable coronary syndrome and sudden cardiac death. Methods: The present study comprised 40 randomly selected healthy male hospital staff without a history of hypertension or cardiac or pulmonary disease. Participants were divided into two groups. The first group included 20 professional smokers (at least 5 pack/year till the time of study) and the second group consisted of 20 non-professional smokers defined as 0.5 pack/ year or less till the time of study. Participants were instructed not to smoke for 6 hours before the study. Patients underwent echocardiography before smoking. The participants were then asked to smoke a whole cigarette. After smoking, echocardiography was repeated within 7 to 15 minutes. Echocardiographic indices of diastolic function (E wave, A wave, Ea, E/A ratio and deceleration time) were measured before and after smoking.Results: There was no statistically significant difference in the baseline measures in both groups before smoking and also there was no significant difference between measures in the two groups after smoking. The analysis of the pooled data from two groups showed that, smoking resulted in significant increase of heart rate (P<0.001). A wave, E wave, Ea, E/A ratio and deceleration time changed significantly after smoking (P<0.001, P=0.027, P=0.011, P<0.001 and P<0.001 respectively). Conclusion: Smoking of only a cigarette in both professional and nonprofessional smokers, resulted in the same significant diastolic dysfunction. The correlation of diastolic dysfunction with TIMI frame count in patients with chronic stable angina pectoris

Abstract: Background: Evaluation of diastolic dysfunction and its relationship with TIMI frame count in patients with stable coronary artery disease referred to Ayatollah Taleghani Hospital in Tehran in the years 2010-2011 is the purpose of this study.Methods: Patients were divided into two groups: case and control. Both groups had chronic angina. Patient information on check list has been studies based on data provided by angiography and echocardiography methods.Results: Cases had significantly higher values of corrected TIMI frame count (TFC) for the left anterior desending artery (LAD), TFC for Circumflex artery (Cx) & Right coronary artery (RCA), (mean TFC P<0.001). Conventional echocardiography showed significantly lower maximal peak systolic velocity (sm) (cm/s), Correlation of early diastolic velocity at myocardial segments (Em) & peak systolic velocity at myocardial segments (Sm) with mean TFC in all 3 vessels were significant (P=0/0001). Ratio of maximal early to late diastolic filling (E/A), in patient with TFC≥ 21 was 0/7 & in patients with TFC≤21 was 1. DT (Deceleration time) had no significant correlation with TFC. Mean of isovolumetric relaxation time (IVRT) in cases was 91 m/s & in controls was 72 m/s Which was higher in patients with mean TFC ≥21. Correlation between mean of angiography and diastolic disfunction was significant.Conclusion: According to this study diastolic dysfunction which estimated by echocardiography showed significant correlation with TIMI frame count in chronic stable angina patients. Due to simplicity, low cost, quality and reproducibility of this method, this will be helpful.
Correlation between Anterior Mitral Leaflet Concavity Area and Ischemic Induced Mitral Regurgitation Severity by Echocardiography

Abstract: Functional mitral regurgitation (MR) results from left ventricular remodeling, leaflet tethering or tenting. Coronary artery disease is one of the important causes of functional MR due to tethering. Detection of functional MR and its severity are important factors in patient prognosis. There are different methods for detection of functional MR and its severity, including anterior mitral leaflet concavity area (AMLCA). In this cross sectional study 32 patients, 19 male with three vessel disease (3VD) who were candidate for coronary artery bypass grafting (CABG) with or without mitral valve replacement were selected. The patients had functional MR in ventriculography. AMLCA was determined by long axis view of transthorasic echocardiography (TTE). In this study 32 patients with 3VD and MR were selected (15 mild, 15 moderate and 2 cases with severe MR). The mean age was 62 ± 12 years. In these patients AMLCA were 0.1- 0.43 cm2 in transthorasic echocardiography. The findings of this study showed strong association between anterior mitral leaflet concavity area and functional MR severity (r=0.89) in parasternal long axis (LAX) view of TTE. According to these findings the relation between functional MR and AMLCA was a linear relationship and increase in severity of functional MR was in concordance with increasing of anterior mitral leaflet concavity area (P=0.001). Mitral leaflet concavity area in the parasternal LAX view provides rapid and reliable recognition of functional MR due to coronary artery disease and is quantitatively related to the degree of such MR. We recommend further study with more subjects and ischemic functional MR and AMLCA.
Intracardiac Shunts and Role of Tissue Doppler Imaging in Diagnosis and Discrimination

Abstract: Background: We sought to assess right ventricular (RV) systolic and diastolic functions via tissue Doppler imaging (TDI) in order to discriminate right-to-left (bidirectional) from left-to-right intracardiac shunts. Methods: A tissue Doppler velocity study via Doppler echocardiography was performed in 20 patients with left-to-right shunt (without evidence of significant pulmonary hypertension) and 20 patients with right-to-left shunt or bidirectional shunt (with significant pulmonary hypertension) or Eisenmenger s complex and 20 healthy subjects as the control group. RV myocardial performance index (MPI), S wave velocity, E wave velocity, isovolumic relaxation time (IVRT), and isovolumic contraction time (IVCT) from the lateral tricuspid annulus were measured using TDI. Results: In the patients with left-to-right shunt, the tissue Doppler parameters showed higher S-wave, peak systolic(Sa)/early contraction(Ea) , Sa/IVRT, and Sa/IVCT values; and in the patients with right-to-left or bidirectional shunt tissue, the Doppler parameters showed higher MPI and MPI/Sa value with a high specificity and sensitivity.Conclusion: We conclude that an evaluation of MPI, S wave, E wave, IVRT, and IVCT via tissue Doppler echocardiography is a useful index for the discrimination of right-to-left from left-to-right and bidirectional intracardiac shunts.
"A COMPARATIVE TRIAL OF LAMOTRIGINE AND CARBAMAZEPINE IN PATIENTS WITH PRIMARY GENERALIZED TONIC CLONIC SEIZURES"

Abstract: Lamotrigine has been used widely in the treatment of partial and secondary generalized seizures. In this study use of lamotrigine as monotherapy for the newly diagnosed primary generalized tonic–clonic seizure has been investigated and compared with carbamazepine. After dose escalation (3 weeks for carbamazepine and 6 weeks for lamotrigin), patients were followed every 4 weeks for the first 16 weeks and then every 8 weeks for the next 32 weeks. Total number of patients was 91, randomly divided in two groups, 46 patients in lamotrigine group and 45 patients in carbamazepine group. The efficacy of the two drugs against primary generalized tonic clonic seizure was almost the same. The proportion of patients with seizure episodes in the last 40 weeks of treatment in both groups was similar (24.2% versus 24.6%). Overall, fewer patients in lamotrigine group than in the carbamazepine group withdrew because of adverse events (6.5% vs. 24.5%, P = 0.0216). The commonest side effect leading to withdrawal with lamotrigine was rash and with carbamazepine was drowsiness. More lamotrigine than carbamazepine recipients completed the study (88.9% vs. 73.3%, P = 0.0961). Lamotrigine and carbamazepine showed similar efficacy against primary eneralized tonic clonic seizure in newly diagnosed epilepsy. Lamotrigine, however, was better tolerated.

Abstract: The aims of the present study were to isolate and serotype, determine the Seroprevalence, Drug susceptibility and diagnosis of infection by Polymerase Chain Reaction (PCR). In this study 460 serum samples and 220 tracheal swabs, 90 ovaries and oviduct swabs, 90 misshapen egg shells swabs were collected from 22 broiler breeder flocks of 5 companies. Serological results showed that all of the 22 flocks (100%) were positive for ORT infection. Ornithobacterium rhinotracheale (ORT) antibodies were detected in 289 (62/83%) out of the 460 serum samples. ORT was detected from tracheal swabs of seven flocks (31/81% or 3/18% out of 220 tracheal swabs). There was significant correlation between flock different ages and ORT titers (p<0.05), but correlation of flock ages and ORT isolates was not significantly different (p>0.05). Seven flocks infected with ORT were detected positive in PCR but bacteria were Isolated from only five culture. No ovaries and oviducts, misshapen egg shell swabs yielded ORT. A 784 bp fragment of the 16S rRNA gene was amplified using ORT specific primers in the PCR. All the isolates were identified as serotype A by Rapid Agglutination Test. Drug sensitivity test using standard disk diffusion technique was performed with 27 antibiotics. Antibiotic susceptibility for Quinolons family was seen more than the others and Cephalosporins family except to Cephalexin. The isolates were 80-100% susceptible to Tetracycline family and the most antibiotic resistant were seen for Aminopenicillins, Polypeptides, Sulfanamides and 80-100% resistant to Aminoglycoside family. Eighty percent of the isolates were resistant to Licomycin and 60% were moderate sensitive to Lincomycin. This study is the first report of prevalence of ORT, bacterial isolation, biochemical characteristics, serotyping and molecular method (PCR) in broiler breeder flocks in Guilan province of Iran.
Bifacial Si Heterojunction-Perovskite Organic-Inorganic Tandem to Produce Highly Efficient Solar Cell

Abstract: As single junction thin-film technologies, both Si heterojunction (HIT) and Perovskite based solar cells promise high efficiencies at low cost. One expects that a tandem cell design with these cells connected in series will improve the efficiency further. Using a self-consistent numerical modeling of optical and transport characteristics, however, we find that a traditional series connected tandem design suffers from low Jsc due to band-gap mismatch and current matching constraints. It requires careful thickness optimization of Perovskite to achieve any noticeable efficiency gain. Specifically, a traditional tandem cell with state-of-the-art HIT (24%) and Perovskite (20%) sub-cells provides only a modest tandem efficiency of ~25%. Instead, we demonstrate that a bifacial HIT/Perovskite tandem design decouples the optoelectronic constraints and provides an innovative path for extraordinary efficiencies. In the bifacial configuration, the same state-of the-art sub-cells achieve a normalized output of 33%, exceeding the bifacial HIT performance at practical albedo reflections. Unlike the traditional design, this bifacial design is relatively insensitive to Perovskite thickness variations, which may translate to simpler manufacture and higher yield.
Bovine Leukemia ProVirus: Evidence of Presence of Part of Gag Gene in Seminal Plasma of Naturally Infected Bulls

Abstract: It is of critical importance to understand the modalities of BLV presence in semen, especially with regard to artificial insemination (AI). Presence of bovine leukemia provirus was demonstrated in fresh and frozen semen samples by researchers. In this study paired blood and semen samples from 45 bulls were assessed for the presence of part of gag gene and antibodies to BLV in blood, semen and cell-free fraction of the semen (seminal plasma). Proviral DNA was detected in 5 out of 45 seminal plasma samples. PCR products were sequenced and submitted to gene bank. This data strongly suggested that seminal plasma of seropositive bulls can be positive in PCR.
Beam Profile and Image Transfer Study in Multimode Optical Fiber Coupling

Abstract: In this study the optical beam transfer and imaging in an optical fiber coupling is discussed. In coupling two fibers, some measurements from intensity perspective are performed and intensity profile is investigated. A theoretical model for the power transmission in fiber coupling is developed and theoretical function is compared with the experimental results. The amount of light delivered to the receiving fiber depends on the acceptance angle of the receiving fiber and the core cross sections of both fibers. Intensity profile function obtained theoretically agrees well with the measured results. The intensity variation as a function of the air gap variation is investigated in this article. The image of emerging beam of the source fiber is also investigated. Using captured images one can measure acceptance angle and core diameter of the fiber by using two images of the output beam at two axial distances. The results of this report can be used in optical communications, data links and image transferring in which single or coupled optical fibers have been used as signal carrier or waveguide.
